Time to submit your paperwork for the 2019-2020 school year!
Heartfelt thanks to the 40% of you who have uploaded all of your required documents. We know everyone is busy and that it takes an investment of time and persistence to get the forms and information we need for our kids. Scans or photos of these items can then be easily uploaded into the SchoolMint account for your child. What you need is:
- Official birth certificate for child
- Immunization record for child
- Parent/guardian driver’s license/state ID
- Proof of residency (a utility bill, mortgage or rent statement with your name and address within the last 30 days)
- If your child currently has one, an IEP or 504 Plan
- If there is a custody agreement, copy of that
