Hello wonderful CSA caregivers!
I am Emma Wiest, The new full-time nurse! I come to you with 15 years of experience in NICU here in Colorado and in my hometown of San Diego. I have three children ranging in age from 3 to 9. I spent last school year at a small school for kids with chronic health conditions in Denver. I’m excited to get to know all the awesome learners here at CSA and work together with you to help provide learners with a safe and healthy environment.
I am in the process of reviewing immunization records. Please note the important information below:
If your learner is missing one or more of the required immunizations and does not have a medical or personal beliefs exemption on file, you will be receiving a letter home along with a list of the state-required immunizations. Many learners are missing their Tdap vaccine. This is a state-required vaccine needed before entering 6th grade. Look to see if your learner needs this.
If your learner has medications at school, we will need a current medical authorization form along with the medications. Please look for this information to come your way if this pertains to your learner. Please keep in mind that if your learner needs medication at school for any length of time, we will need medical authorization on file.
If your learner has asthma and/or a life-threatening allergy we should have a current asthma action plan and/or anaphylaxis plan on file in the clinic. These forms can be found in the office.
If your learner has a chronic medical condition or a health concern and we have not spoken yet, please email or call me so that we can set up a time to discuss your learner’s needs.
